Video Release

I grant permission to Choosing New and its agents and employees, a) to record and capture my image, likeness and voice on the date(s) and at the location(s) listed below on video, audio, photographic, digital, electronic or any other medium (the Recordings) and to use my name, title and biographical information in connection with the Recordings without limitation; and b) to use, reproduce, exhibit or distribute the Recordings in whole or in part irrevocably and in perpetuity in any and all media throughout the universe (including but not limited to print publications, digital recordings, online and any other electronic or other medium now known or later developed) for any purpose that Choosing New, Inc. deems appropriate, including promotional, recruiting, advertising and any commercial or non-commercial use, without limitation.

I waive any right to inspect or approve the Recordings, or any uses thereof, now or in the future, and I waive any right to royalties or other compensation arising from or related to the use of the Recordings. I understand that the Recordings, in whatever medium, will be owned by Choosing New, Inc.

I release and discharge Choosing New, Inc of and from any claims, demands and damages that may arise from or related to the use of my name, image, and likeness in the Recordings, including any claims for libel or violation of any right of publicity or privacy and including any re-use, distortion, blurring, alteration or use in composite form. It is in the discretion of Choosing New, Inc to decide whether and how to use the Recordings.

This Release will be binding upon me and my heirs, legal representatives and assigns.

Unless my parent or guardian signs where indicated on the signature lines below, I certify that I am 18 years of age or older and I am competent to contract in my own name. I have read this Release and I fully understand the contents, meaning and impact of this Release.

For subjects of the Recordings who are under 18, this Release must be signed by both the minor subject and their parent or guardian. By signing, the parent or guardian attests that they are competent to contract in their own name, have read this Release, and fully understand the contents, meaning and impact of this Release.
